Most osteosarcomas are high-grade and require multimodal treatment, mainly chemotherapy, and surgery. Low-grade osteosarcoma (including parosteal and low-grade central) does not require chemotherapy, but if the histological examination of a resected tumor shows high-grade areas, chemotherapy is recommended. The most common combinations of chemotherapy include high-dose methotrexate, doxorubicin, and cisplatin (sometimes with ifosfamide); doxorubicin and cisplatin; ifosfamide and etoposide; and ifosfamide, cisplatin (or carboplatin), and epirubicin.
